Accident / Incident
Reporting investigation and Analysis are a critical element of
safety management. The overriding purpose for an organization in carrying out
incident investigation is prevention of similar incidents as well as seeking a
general improvement in the management of health and safety. Statistics
confirms the general experience that in two accidents out of three the
cause is blamed on the person carrying out the task. A more careful
investigation should have revealed that the accidents were caused by a “lack of
management control” due to inadequacies in the SH&E programme, standards,
compliance, monitoring of work, etc.
Accident

Accident / incident reporting, investigation
and analysis is an important facet of safety in any industry. Apart from being
a legal requirement under various statutes, the activity helps in finding out
the root causes of the accident / incident based on which the CAPA (corrective
and preventive actions) can be decided.
The three primary tasks of the accident investigator are to
gather useful information, analyze the facts surrounding the accident, and
write the accident report. The intent of this workshop is to help you gain the
basic skills necessary to conduct an effective accident investigation at your
workplace. Only experience will give you the expertise to fine-tune those
skills.
Most of the
information about conducting an accident investigation will come directly from
the class as we discuss issues, answer basic questions and complete group
activities. If you have prior experience in accident investigation, we hope you
will participate actively so others may benefit from your valuable input.
Ultimately, we want you to leave this workshop knowing how to conduct an
accident investigation and properly complete an accident investigation report
with confidence using systematic
approach.
The
activity is also an important element of OHSAS – 18001 audit system
SCOPE AND PURPOSE
The purpose of this workshop is to discuss the key elements of
incident/accident investigation, root cause analysis and developing corrective
actions to eliminate similar incidents reoccurring at our workplace thus making
a incident/accident free environment
WHEN DO YOU CONDUCT AN INVESTIGATION?
All incidents, whether a near miss or an actual injury-related event,
should be investigated. Near miss reporting and investigation allow you to
identify and control hazards before they cause a more serious incident.
Accident/incident investigations are a tool for uncovering hazards that
either were missed earlier or have managed to slip out of the controls planned
for them. It is useful only when done with the aim of discovering every
contributing factor to the accident/incident to "foolproof" the
condition and/or activity and prevent future occurrences. In other words, your
objective is to identify root causes, not to primarily set blame.
WHO
SHOULD CONDUCT INVESTIGATION?
Ideally, an investigation
would be conducted by someone experienced in accident causation, experienced in
investigative techniques, fully knowledgeable of the work processes,
procedures, persons, and industrial relations environment of a particular
situation.
Some jurisdictions provide guidance such as
requiring that it must be conducted jointly, with both management and labour
represented, or that the investigators must be knowledgeable about the work
processes involved.
In
most cases, the supervisor should help investigate the event. Other members of
the team can include:

employees with knowledge of the work (
supervisor)
·
safety officer
·
health and safety committee
·
union representative
·
employees with experience in
investigations
·
"outside" expert
·
representative from local government
In
view of above officials should attend the workshop.
THE
FACULTY
The
workshop will be conducted by Mr. D. K. Joshi. Mr. Joshi is a well-known
professional in the field of Industrial Safety. Mr. Joshi is basically a
Chemical Engineer from Bombay University Department of Chemical Technology. He
worked in Hazardous Process Industry for 23 years in different capacities. He is a lead auditor for OHSAS – 18001. For the last several years, Mr. Joshi is very
active in the field of Industrial Safety. He has conducted Safety Audits, Hazop
Studies and Training Programs on various Safety related topics. He has
delivered several lectures in public programs on Industrial Safety.
Mr. Joshi is a regular faculty for
the Advanced Diploma Course in Industrial Safety. He is a member of different
institutions such as GTS, N.S.C., L.P.A., I.I.S.P.I., Institute of Energy
Management etc.
